Each document should be a separate PDF. Exhibits should be filed as attachments to the document to which they relate. See Electronic Filing Process. A document which exists only in paper format should be scanned.

If you are filing a transcript as an exhibit and have an ASCII version, you can convert it to PDF through your word processing program.  See Convert a Document to PDF.
